I attest that I practice prenatal, postpartum, and/or pediatric care with the highest standards of integrity, professionalism, and ethical responsibility.
I commit to providing compassionate, evidence-based care for all patients, prioritizing their health, safety, and well-being for all prenatal, postpartum, and/or pediatric care. In doing so, I adhere to the following principles:
- Honesty and Transparency: I maintain open communication with my patients, ensuring they are fully informed about their care options, treatment plans, and any risks involved in their prenatal, postpartum, and/or pediatric care.
- Respect and Dignity: I treat all patients with respect, dignity, and empathy, fostering a supportive and trusting environment for individuals and families during a critical phase of life.
- Confidentiality: I uphold the confidentiality of patient information in compliance with all relevant legal and ethical standards, ensuring patient privacy is never compromised.
- Continuous Professional Development: I remain committed to ongoing education and staying informed of the latest research and best practices in prenatal, postpartum, and/or pediatric care to provide the most up-to-date and effective care.

Terms of Service for Provider Membership on LoveStrong Birth & Postpartum Resource Guide
By applying for and maintaining a listing on San Diego Birth & Postpartum, all providers attest to the above commitments to patient care and agree to the following Terms of Service
- Nature of the Relationship
Providers listed on the Resource Guide are independent practitioners. Inclusion in the Resource Guide does not create an employment relationship, agency, partnership, joint venture, or legal affiliation with LoveStrong Wellness. LoveStrong Wellness does not supervise, direct, or control the clinical services provided by any listed Provider. - Purpose of the Resource Guide
The Resource Guide exists to help families discover prenatal, postpartum, and pediatric support resources. Inclusion is based on provider self-attestation and alignment with shared values. Listing in the Resource Guide does not constitute a medical, clinical, or professional endorsement by LoveStrong Wellness. - Provider Responsibility & Self-Attestation
By applying, Providers attest that:- All credentials, licensure, certifications, and representations are accurate and current.
- Services are provided within their professional scope of practice and in accordance with applicable laws and regulations.
- They are solely responsible for the care they provide and for maintaining compliance with professional and legal standardsProviders agree to promptly update LoveStrong Wellness if theri credentials, scope, or practice status changes.
- Accountability and Removal Policy
Failure to uphold any of the above standards may result in removal from the LoveStrong Birth & Postpartum Resource Guide. Concerns or reports regarding a provider’s conduct will be reviewed on a case-by-case basis. If, after review, a provider is found to be in violation of these terms, their membership and listing may be suspended or permanently removed at the discretion of the site administrators.
By remaining listed on the Resource Guide, providers attest their commitment to these values and agree to be held accountable to these standards of integrity and professionalism. - No Clinical Oversight or Guarantee
LoveStrong Wellness does not provide medical advice, supervise clinical care, or guarantee outcomes. Families are responsible for selecting providers that meet their individual needs. LoveStrong Wellness is not liable for services rendered by Providers in the Resource Guide. - Membership Term & Renewal
Membership in the Resource Guide is offered on an annual basis. Payment is due at the time of application and is required for review and inclusion in the Resource Guide.
All membership fees are non-refundable, including but not limited to situations involving voluntary withdrawal, failure to renew, removal due to violation of these Terms of Service, or removal for conduct inconsistent with the Resource Guide’s states values and attestations.
Continued inclusion in the Resource Guide requires timely annual renewal and payment of applicable membership fees. Failure to renew may result in removal from the Resource Guide and associated platforms. - Website & Social Media Inclusion
Membership may include:- A listing on the Resource Guide website.
- Participation in the dedicated Resource Guide social media or community platforms.
- Participation in online platforms is a privilege. LoveStrong Wellness reserves the right to moderate content and remove Providers from these spaces at its discretion to maintain a respectful, evidence-based, and values-aligned environment.
- Right to Modify or Remove Listings
LoveStrong Wellness reserves the right to edit, suspend, or remove any listing at its discretion, with loss of memebership fees, for reasons including but not limited to cases of:- Misrepresentation or inaccurate information
- Violation of stated attestations or values
- Conduct that may harm families, other providers, or the integrity of the Resource Guide.
- Use of Name & Branding
Providers may not represent themselves as partners, affiliates, or representative of LoveStrong Wellness. Use of LoveStrong Wellness branding, logos, or language implying endorsement or partnership is prohibited without prior written permission. - Limitation of Liability
LoveStrong Wellness shall not e liable for any claims, damages, losses, or disputes arising from services provided by listed Providers. Any liability related to Resource Guide Membership is limited to the amount of membership fees paid. - Indemnification
Providers agree to indemnify and hold harmless LoveStrong Wellness, its owners, and affiliates from any claims, liabilities, damages, or expenses arising from their professional services, representations, or conduct. - Modification to Terms
LoveStrong Wellness may update these Terms of Service as needed. Continued participation in the Resource Guide constitutes acceptance of any updated terms. - Governing Law
These Terms are governed by the laws of CA in which LoveStrong Wellness operates.
By submitting an application or maintaining a listing, Providers acknowledge that they have read, understood, and agree to these Attestations and Terms of Service.
